• Plan and administer prescribed skilled physical therapy treatment and training for patients with various injuries, illnesses and functional disabilities to attain the highest level of physical function.
• Screen the patient’s physical strengths to assist the physician in evaluating functional level and document findings to develop or pursue treatment programs with measurable objectives.
• Develop and implement a conditioning/rehabilitation program aligned with the physician s Plan of Treatment and patient goals; adjust treatment as needed to maximize results.
• Collaborate with physician and clinical team to obtain patient information and assist in developing, implementing and revising the therapy program and Plan of Treatment.
• Provide supervision and written care plans to Physical Therapy Assistants and Home Health Aides; monitor documentation and progress.
• Document patient care observations, interventions and evaluations accurately and promptly; ensure orders are documented and submitted for physician signature as required.
• Report patient progress to the physician, Clinical Manager, staff, patient and family; submit evaluation, treatment plans, progress reports and discharge summaries as applicable.

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Minimum of one year physical therapy experience preferred
• Current and unrestricted Physical Therapy license
• Current CPR certification
• Strong organizational and communication skills
• A valid driver’s license, auto insurance, and reliable transportation are required

• Schedule:
  
  40 hours per week
• Pay range (base): $85,400 - $117,500 per year
• Pay per visit/unit option: $54.00 - $76.00
