Licensed Practical Nurse – Home Health
Center Well Home Health is seeking a dedicated Licensed Practical Nurse (LPN) to provide high‑quality, personalized care to patients in the comfort of their homes. This full‑time, 40‑hour per week position offers a competitive salary and comprehensive benefits.
Responsibilities- Provide professional nursing services under the direction of an RN, in compliance with state and company policies.
- Administer skilled nursing interventions, promote preventive care, and use holistic approaches to support patient recovery.
- Develop and manage individualized care plans with physicians, therapists, and family members.
- Communicate clearly with the RN supervisor, physicians, and interdisciplinary team, including verbal orders as permitted by law.
- Maintain accurate, timely documentation and shift notes in the company’s system.
- Ensure confidentiality and professional conduct in handling patient information.
- Coordinate care activities and discharge planning as needed.
- Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver’s license, reliable transportation, and auto insurance.
- Current CPR certification.
- At least two years of LPN/LVN experience in a clinical setting, preferably in home health or hospice.
Base pay: $44,600.00 – $61,400.00 per year. Pay per visit/unit: $28.00 – $40.00
