As an Independent Optometrist leasing space inside a Sam’s Club, you will take a leading role in patient care with respect to eye and vision care as well as general health and well‑being.
Position SummarySalary:
Based on experience.
Schedule:
Full‑time.
- Lead patient care for eye and vision services.
- Establish your days of practice, hours of operation, and patient schedule.
- Set your fee schedule and retain 100% of exam/professional fees.
- Equipment/instrumentation
- Administrative and/or Opt‑Tech support services (if state allows)
- Advanced Equipment/instrumentation
- Doctor of Optometry (OD) from an accredited optometry school
- Valid state optometry license
- Ophthalmologist (MD or DO) from an accredited school
- Valid state ophthalmology license
- General Liability Certificate
- Professional Liability Insurance Certificate
- W‑9
ME & VA license agreements are with Gumberg Asset Management Company.
