Independent Optometrist – Sam’s Club — Launch your services in Sam’s Clubs at 7700 Rogers Ave, Fort Smith, AR , United States of America. As an Independent Optometrist leasing space inside a Sam’s Club, you will take a leading role in patient care with respect to eye and vision care as well as general health and well‑being.

Salary:
Based on experience.

Schedule:

Full‑time.

• Lead patient care for eye and vision services.
• Establish your days of practice, hours of operation, and patient schedule.
• Set your fee schedule and retain 100% of exam/professional fees.

• Equipment/instrumentation
• Administrative and/or Opt‑Tech support services (if state allows)
• Advanced Equipment/instrumentation

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W‑9
