MDS RN Nurse

Springs of Covington, a leading Skilled Nursing Consulting Company, is seeking a dedicated and compassionate MDS RN Nurse to join our team in Fort Smith, AR. As a vital member of our care team, you will be responsible for providing high-quality care to our cherished residents in our Skilled Nursing facilities across Arkansas.

• Conduct comprehensive assessments and develop individualized care plans for residents in accordance with federal and state regulations
• Collaborate with interdisciplinary teams to ensure seamless care transitions and achieve optimal outcomes
• Coordinate and facilitate care planning meetings, educating residents, families, and care team members on care plans and progress
• Utilize electronic medical records to document accurate and timely assessments, care plans, and progress notes
• Stay current with industry developments, best practices, and regulatory requirements, applying this knowledge to improve care delivery and outcomes

• Minimum 1 years of experience as an RN in a Skilled Nursing facility; prior experience as an MDS Coordinator or RN Case Manager a plus
• Active RN license in the state of Arkansas
• Excellent communication, interpersonal, and leadership skills
