Parkview Physicians Group is currently seeking experienced Board Eligible / Board Certified
Family Medicine or Internal Medicine physicians to join our Transitional Care Clinic team in Fort Wayne, Indiana
.
- Part-Time role - 20 hours per week to include rounds Monday - Friday
- Involves travel by conducting daily rounding on Parkview patients in high performing network skilled nursing facilities
- Nurse practitioners cover call; physician would need to be available as a backup by phone
- Group consists of 2 physicians & 5 advanced practice providers that provide quality continuation of care to patients that are discharged from the hospital
- Comprehensive care team and clinical support model comprised of medical assistants and registered nurses
