What You’ll Do as a Home Health Occupational Therapist:

  • Provide skilled occupational therapy to patients in their homes based on individualized treatment plans.
  • Develop purposeful activities and interventions designed to improve physical, emotional, and cognitive function.
  • Evaluate patient progress and revise plans of care as necessary to achieve maximum functional independence.
  • Educate patients, families, and caregivers on adaptive techniques and equipment to support daily living and safety.
  • Collaborate with physicians, nurses, physical therapists, and other interdisciplinary team members to ensure well-coordinated, high-quality care.
  • Document all care, assessments, and interventions clearly and in a timely manner.
  • Support patients as they work to regain the skills needed for dressing, grooming, cooking, bathing, and other essential daily activities.
About You:

Qualifications – What You’ll Bring:

  • Bachelor’s or Master’s degree in Occupational Therapy from an accredited institution.
  • Current OT license in the state of employment and in good standing with the appropriate state board.
  • Ability to work independently and adapt to varied home environments.
  • Strong clinical, communication, and problem-solving skills.
  • Valid driver’s license, auto insurance, and reliable transportation.

Preferred Experience (Not Required):

  • 1+ year of occupational therapy experience
  • Home health or community-based rehab
  • Geriatric, neurological, psychiatric, or cardiopulmonary therapy
  • Interdisciplinary team collaboration
