Medical Scribe – Oak Street Health, part of CVS Health (Fort Wayne, IN) – a full‑time role focused on supporting primary care providers with clinical documentation to ensure exceptional patient care.
About the RoleThe Clinical Informatics Specialist (CIS/Medical Scribe) assists providers throughout the patient care journey, from morning huddles to exam room documentation and after‑visit follow‑up. Scribes are integral to accurate, timely, and value‑based documentation, including ICD‑10 and CPT coding, and help close care gaps in chronic condition management.
Responsibilities- Documenting Patient Encounters ~ 80%
- Joining the provider in the exam room to observe patient visits
- Documenting encounters in a structured note (history of present illness, assessment, plan, physical exam)
- Assigning appropriate CPT and ICD‑10 codes
- Preparing After Visit Summaries
- Consulting with the provider to ensure accurate documentation
- Clinical Documentation Improvement ~ 10%
- Requesting and reviewing medical records
- Leveraging Oak Street's population health tools to support documentation improvement
- Preparing for and supporting Daily Huddles and Clinical Documentation Reviews
- Consulting with provider on documentation opportunities
- Administrative support for your provider and care team ~ 10%
- Placing orders and referrals
- Addressing tasks
- Supporting the care team with additional responsibilities related to clinical documentation
- Other duties as assigned
- Medical terminology and common medications (pre‑med or prior clinical experience) –
required - Prior clinical experience, including shadowing and/or volunteering –
strongly preferred - Prior scribe or transcription experience –
preferred but not required
- Advanced listening and communication skills –
required - Strong computer literacy and ability to learn new technical workflows –
required - Fluency in Spanish, Polish, Russian, or other languages spoken by community members –
required where indicated
- Adapt to new workflows and quickly learn concepts and skills –
required - Typing 70+ words per minute –
strongly preferred - Take direction and work as part of a patient‑care team –
required - Self‑starter within role scope
- Excellent attendance and willingness to work in‑person in clinics –
required - Commit to at least 1 year in role (2+ ideal) –
required - Work approx. 40‑45 hours per week during clinic hours –
required - Compliance with HIPAA and hospital policies –
required - U.S. work authorization –

Pay Range: $17.00 - $25.65 per hour
