Community Home Health Care is growing—and we’re looking for passionate case managers to join our team. We also have an immediate opening for a Bilingual Case Manager (English/Burmese).

What You’ll Do

• Client Assessments & Care Plans – Evaluate needs and design personalized care plans
• Coordinate Services – Partner with nurses, caregivers, and providers for seamless support
• Advocate for Clients – Be a trusted voice for patients and families
• Maintain Compliance – Keep records accurate and services aligned with state and federal guidelines
• Crisis Support – Step in quickly when urgent issues arise
• Monitor & Adjust – Track progress and adapt care plans to improve outcomes

What We’re Looking For

• Excellent communication & relationship-building skills
• Strong organization and time-management abilities
• Knowledge of Medicaid, Medicare, and Indiana home care guidelines
• Background in case management, social work, or healthcare
• Tech-savvy with EHR or case management systems
• Bilingual (English/Burmese) is a plus—immediate need!
