Hours:
Monday–Thursday, primarily 8:30 a.m.–4:30 p.m.; one evening per week required with adjusted daytime hours; 28–32 hours/week; participation in a shared night/weekend on‑call rotation; occasional overtime as needed.

The Case Manager plays a vital role in supporting women in Hope Alive’s Residential Program. This position provides individualized case management, advocacy, and resource coordination to help clients achieve stability, develop life skills, and move toward greater independence and hope.
Key Responsibilities Client Support & Case Management- Assess client needs, strengths, and barriers and develop individualized care plans
- Provide ongoing, compassionate support related to mental health, housing, employment, and daily living skills
- Collaborate with clients to set goals and monitor progress
- Facilitate groups and classes as needed
- Connect clients with internal and community resources, including healthcare, housing, employment, transportation, and social services
- Advocate for clients within community systems and assist with applications and navigation of services
- Maintain timely, accurate client documentation, including care plans and progress notes
- Complete reports and documentation required for grants and program accountability
- Work closely with counseling and residential staff to provide integrated care
