Wellness Nurse

Silver Birch of Fort Wayne Cook Road is seeking a Wellness Nurse to join our Clinical Team. Silver Birch Nurses provide exceptional care and service to our residents. We are seeking a Licensed Practical Nurse (LPN) that will have a fulfilling career with the focus of providing the best care to our residents.

$29.00/hr - $29.00/hr

• Manages medication orders and documentation in compliance with regulations
• Follows up with service providers to ensure resident needs are met
• Administers medications and treatments as ordered and documents
• Initiates and documents care delivered and care coordination, including completing admissions, assessments, transfers, and discharges
• Support and educate residents and families on proper healthcare directed by the resident care plan and promote compliance
• Provides exceptional customer service
• Oversight of QMA and CNA and support in resident care

• High School Diploma/GED
• Current LPN Licensure"+ "
• Experience in Healthcare
• Two or more years working with the senior population (preferred)
• Exceptional Customer Service Skills
