Registered Nurse; RN or Licensed Practical Nurses; LPN - 3rd shift

• Wednesday: 12:00 a.m. – 6:00 a.m.
• Thursday: 6:00 p.m. – 6:00 a.m.
• Friday: 6:00 p.m. – 6:00 a.m.
• Saturday: 6:00 p.m. – 6:00 a.m.

• Be responsible for the care and well‑being of the residents in your care
• Assess the residents’ needs and assist in developing and following individual care plans
• Assure quality care is delivered through the utilization of nursing processes and guidelines
• Identify and communicate changes in residents’ status to physicians, staff, and family members
• Administer medications, treatments, and other direct care as prescribed and indicated
• Coordinate admissions, transfers, and discharges of residents
• Communicate appropriate information to all staff to provide a smooth shift transition
• Investigate and document incidents and accidents as required
• Report any suspected abuse to appropriate authority
• Document appropriately as needed or required in PCC
