About The Role

Longevity Health is looking for a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long‑term care settings. In this role, you will be responsible for a panel of enrolled members receiving nursing home care, conduct comprehensive assessments, develop individualized care plans, actively participate in nursing facility care team meetings, maintain frequent communication with families, and ensure the care team remains aligned with members’ goals and needs.

Tasks and Responsibilities
  • Provide collaborative primary care and services to a panel of Longevity Health Services, treating in the most appropriate setting based on the member’s goals of care.
  • Execute and deliver contracted plan’s activities, develop and implement advanced care planning, and embrace a treating‑in‑place model.
  • Facilitate, administer health plan clinical operation functions—prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promote membership growth.
  • Serve as a trusted advisor to members, loved ones, nursing home staff, and member advocate, educate care teams on member needs and early identification of changes, and notify APP and telehealth partners for member changes.
  • Deliver effective and frequent communication and coordination with members’ PCPs, loved ones, and care team members on members’ care needs, goals of care, treatment plans/changes.
  • Support overall market goals/targets and balance member caseload demands with other team member support needs.
  • Promote Longevity Health with members, loved ones, and nursing facilities and other stakeholders.
  • Function autonomously and be accountable for practice.
  • Establish meaningful relationships and function in the nursing home environment.
  • Be member‑centric and customer‑service oriented, establishing meaningful relationships that ensure effective and high‑quality care for Longevity Health members.
  • Perform other related duties as assigned.
Supervisory Responsibilities
  • There are no supervisory responsibilities for this position.
Credentials & Coverage
  • State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner; acute care nurse practitioner experience with nursing home/geriatric or hospice palliative care OR state unrestricted licensed Physician’s Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
  • Active DEA license registered in the state of practice.
  • Active national provider number (NPI) and an active Medicare provider number.
  • Eligibility to participate in the Medicare program as a provider and registered.
  • Valid state driver’s license with a good driving record and proof of automobile insurance.
  • Auto‑insurance coverage as required by state. New employees must provide proof of coverage prior to hire and on an ongoing basis, and be willing to drive up to one hour to and from work daily.
  • Must be present in SNF communities/buildings during business hours daily.
Education And Training
  • Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program.
Knowledge And Experience
  • Minimum of six months experience as a Nurse Practitioner/Physician’s Assistant AND some experience with geriatric/hospice or palliative care population OR a licensed and board‑certified new graduate NP/PA supervised by an experienced NP/PA in the LHP New Graduate Fellowship program for six months and successfully completing all competencies for independent practice as an LHP APP.
  • Experience working in the nursing home environment is strongly desired.
  • Computer/technology proficiency required.
  • Excellent communication, people, and clinical skills for assessment, diagnosis, treatment, and prescribing to the chronically ill population.
  • Self‑directed and functions well in a team environment.
  • Demonstrated critical thinking, organization, and prioritization skills.
