Licensed Practical Nurse (LPN) | Skilled Nursing, Rehab & Long-Term Care | Kingston Care Center of Fort Wayne, IN

What You’ll Do

Deliver skilled nursing care in alignment with individualized resident care plans

Administer medications and treatments accurately and safely

Monitor and document resident conditions, reporting promptly to RNs and physicians

Collaborate with interdisciplinary teams to ensure quality outcomes and resident well-being

Provide guidance and leadership to CNAs to maintain high standards of care

Help foster a safe, clean, and supportive environment for residents and staff

Qualifications

Active Licensed Practical Nurse (LPN) license in Indiana (required)

CPR certification required

Experience in skilled nursing, rehab, or long-term care preferred

Strong clinical judgment and attention to detail

Compassionate, reliable, and team-oriented personality

Excellent communication and interpersonal skills
