As an LPN/LVN, you will provide compassionate home‑care to patients, working closely with registered nurses and other healthcare professionals to achieve outstanding clinical outcomes. Your responsibilities will include bedside nursing care, medication administration, documentation, and collaborating with the care team to develop individualized care plans.

• Active, unencumbered LPN/LVN license in the state of practice
• Compact licenses must be transferred to your state of residence within 90 days
• Current CPR certification (hands‑on component) – assistance available after hire if necessary
• Valid TB skin test (within last 12 months)
• Six months of hands‑on nursing experience preferred but not required
• Reliable transportation
