- Provide skilled nursing care to residents, including administering medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans to meet the unique needs of each resident
- Collaborate with interdisciplinary teams to ensure comprehensive care and support
- Provide emotional support and comfort to residents and their families
- Maintain accurate and detailed records of resident care and progress
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
- Current LPN licensure in the state of Indiana
- 1+ year of experience in long-term care or a related setting
- Ability to work effectively in a fast-paced environment with minimal supervision
- Excellent communication and interpersonal skills
- Compassion and empathy for residents and their families
