About the Role

Make a meaningful difference in the lives of patients by providing compassionate, hands-on care in the place they call home. As a Home Health or Hospice Aide with Optimal Care, you'll help patients maintain comfort, dignity, and quality of life while supporting their individual needs and goals. Whether assisting patients on their journey toward recovery and greater independence or providing comfort and reassurance during end-of-life care, you'll play an essential role in their daily well-being.

This role is ideal for a compassionate caregiver who enjoys building meaningful relationships and making a difference through personalized care. You'll work closely with patients, families, nurses, and other members of the interdisciplinary care team to support individualized care plans and respond to each patient's changing needs. Every visit is an opportunity to provide encouragement, celebrate progress, offer reassurance, and create moments of comfort and connection.
