Post Diagnostic Support Link Worker – NHS Highland

Applications are welcome for a fixed‑term, 9‑month contract to support adults living with dementia as part of the Older Adult Team in Lochaber, working alongside the National Alzheimer’s Scotland Team.

• Provide post‑diagnosis support to adults living with dementia, including planning for the future and developing outcome‑focused care plans with clients and their families.
• Collaborate with a multidisciplinary team and the Lochaber Community Mental Health Team.
• Maintain accurate records and use IT systems effectively.

• Experience working with older adults or in mental health settings.
• Minimum SVQ Level 4 in Health and Social Care, SQA Level 9, or an equivalent current professional qualification in health care.
• Full UK driving licence and availability to use own vehicle for work purposes.
• Excellent communication skills, good IT proficiency and team‑working abilities.

• This role does not meet the eligibility criteria for a Skilled Worker Visa or a Health and Care Worker Visa, and NHS Highland is unable to provide a Certificate of Sponsorship.
