Job Description
:
The Residency program aims to develop clinical pharmacists responsible for medication‑related care of patients with a wide range of conditions. The 12‑month training program meets ASHP guidelines, with residents working closely with experiential preceptors during various learning experiences. Residents coordinate activities, monitor performance, and complete projects.
- Develop and optimize pharmacotherapeutic regimens.
- Work closely with medical staff and multidisciplinary teams to identify drug therapy alternatives and ensure optimal patient care.
- Participate in interdisciplinary patient care rounds and follow up with the clinical team.
- Provide concise, applicable, comprehensive, and timely responses to drug‑information requests from patients and health care providers.
- Facilitate medication reconciliation by performing drug‑utilization review, monitoring for drug interactions, minimizing polypharmacy and preventing potential adverse events.
- Actively work with and educate patients on medications, engaging them in understanding their medication regimens.
- Participate in required pharmacy standards and adhere to all state and federal regulations.
- Maintain patient confidentiality and employee information per policies and procedures.
- Complete a major project.
- Ensure completion and accuracy of required documentation and record keeping.
- Represent the department in interdepartmental and district meetings as required.
- Participate in staffing components outlined in the residency program.
- Comply with all ASHP residency requirements and JPS resident manual policies.
- Perform other job‑related duties as assigned.
Required Education and Experience
- Doctorate of Pharmacy (Pharm.
D.) degree from an accredited College of Pharmacy. - PGY2 – Successful completion of 1 year residency through ASHP.
Required Licensure/Certification
- PGY2 must have current licensure from the Texas State Board of Pharmacy by July 1 of the current residency year.
- Must maintain licensure throughout residency.
Preferred Education and Experience
- Documented history of research and/or publication experience.
- Leadership experience in a professional organization.
- Participation in community service projects.
Preferred Licensure/Certification
- Basic Life Support Certification.
- Immunization Certification.
