Community Healthcare of Texas is currently recruiting for a Social Worker.
Position SummaryTo provide therapeutic/emotional support, education, and resource information to patients and their families;
Advocate for patient’s needs/wishes with team members, family members, caregivers, and other providers.
- Assess the psychosocial needs of the patient and family members
- Provide therapeutic counseling and emotional support to patients and family members and education on end of life issues
- Contribute to and execute the Plan of Care with family/patient
- Record daily documentation of visits and contact in accordance with Policy
- Provides referrals to community resources
- Perform other duties as assigned
- Bachelors of Social Work with 1 year of applicable experience required;
Master's degree preferred - Reliable transportation and valid driver's license.
- Hospice or healthcare experience preferred.
- Proficient skill using a computer is required
