• Provide clinical pharmacy support to patient care teams.
• Review and reconcile medication orders, ensuring safety and efficacy.
• Participate in interdisciplinary rounds, medication therapy management, and adverse drug reaction monitoring.
• Conduct IV infusion preparations and quality assurance.
• Educate patients, families, and staff on medication therapy.

• B.S. or Pharm.
  
  D. from an ACPE-accredited School of Pharmacy.
• Minimum 1 year as a Clinical Pharmacist in a hospital setting or completion of a specialty residency or fellowship.
• Active pharmacist license in good standing with the appropriate State Board of Pharmacy.
• ACPE accredited IV certification.
• Specialty BCPS certification.
