• Works to create connections between vulnerable populations and AHF healthcare center providers.
• Conducts extensive outreach to re‑engage at‑risk patients back into care.
• Retains patients in care to achieve desirable health outcomes.
• Provides high quality, patient‑facing service and advocates for AHF’s top initiatives.
• Ensures AHF Core Values and Mission stay top of mind with all that you do.
• Works as part of a close‑knit team to keep patients engaged and helps navigate and eliminate barriers to care.

• High school/equivalency diploma required.
• Associate/Bachelor’s degree in Public Health, community health, social services, or related field preferred.
• Three years related experience and/or training; or equivalent combination of education and experience.
