Position SummaryThe Patient Navigator – Grants promotes Patient and Family Centered Care principles as patients, families, staff, and providers navigate through JPS Health Network. The role involves providing benefits counseling, facilitating access to resources, and enhancing patient experience.
Essential Job Functionsиваться & Responsibilities- Listen to patients’ and families’ feedback, determine appropriate health team member or department, and facilitate resolution.
- Communicate policies, protocols, rights and responsibilities to patients, families, and visitors; coach staff on culturally appropriate communication.
- Promote and recommend process changes to improve overall patient and family experience.
- Implement educational programs supporting Patient & Family Centered Care principles.
- Build professional relationships with community organizations and promote JPS services.
- Recognize quality improvement opportunities and initiate processes to meet patient/family needs.
- Provide benefits/entitlement counseling and referral to health services for public and private programs.
- Educate patients about benefit programs, assess eligibility, assist with applications, re‑certifications, and advocacy in appeals.
- Initiate and follow up on referrals to agency and community service providers.
- Develop and maintain effective working relationships with area social service providers and coordinate activities to ensure integrated service delivery.
- Document service delivery and maintain patient records with confidentiality per JPS Policies & Procedures.
- Attend agency-related meetings, retreats, work sessions, and events as requested.
- Perform other duties as assigned, as the description may change.
- High School Diploma, GED, or equivalent.
- At least three (3) years of experience in a customer service environment.
- For HIV Grants: complete all training required of Agency new hires and TDSHS HIV Care Services Branch Standards of Care within the established time frames.
- For SUD Grants: obtain Community Health Worker Certification or Recovery Support Peer Specialist Certification within one year of employment.
- For Maternal Health Grants: obtain Community Health Worker Certification within one year of employment.
