My Health My Resources of Tarrant County (MHMR) is looking for a skilled Service Coordinator. Each person receiving services in the HCS, TxHmL, and GR CFC in Tarrant County will have a single identified staff, called a Service Coordinator (SC). The SC will assist the person in identifying and articulating his/her priority outcomes, personal goals, and hopes for the future as well as his/her requests and needs for services and supports.
The SC will elicit the person's feedback, coordinate, monitor the services for desired outcomes as identified in the Person Directed Plan (PDP). The goal of a Service Coordinator is to provide the people we serve with seamless consumer-driven services and support. The SC must also be knowledgeable about the array of services and supports available in the community.
Bachelor's degree in applied Behavior Analysis, Child Development, Counseling, Early Childhood Education, Early Childhood Intervention, Education, Psychology, Gerontology, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Registered Nurse, Rehabilitation & Rehab Studies, Social Work, Sociology, or Special Education
Preferences:Spanish fluency
Substitutions:Any bachelor's degree with completion of college level coursework equivalent to requirements for QMHP credential
License/Certifications:
Qualified Mental Health Professional (QMHP) OR eligible for QMHP certification (based on minimum and defined education above);
Valid Texas Driver’s License
