Cardiology Nurse Practitioner​/Physician Assistant

We are seeking a friendly and compassionate Cardiology Nurse Practitioner or Physician Assistant to join our team. As a Cardiology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with heart-related conditions. Fort Worth, TX

Part-Time (3 days per week)
Salary: $90,000.00 - $

• Perform comprehensive patient assessments, including physical examinations, medical histories, and diagnostic tests.
• Collaborate with cardiologists to develop and implement treatment plans for patients with cardiovascular conditions.
• Administer medications, monitor patient responses, and adjust treatment plans as necessary.
• Conduct cardiac stress tests, electrocardiograms (EKGs), and other diagnostic procedures.
• Educate patients and their families about cardiovascular health, preventive measures, and the importance of medication adherence.
• Provide counseling and emotional support to patients and their families.
• Medication Management:
  Ensure residents receive the appropriate medications, monitor for side effects, and adjust dosages as needed.
• Chronic Disease Management:
  Provide ongoing care for residents with chronic conditions such as diabetes, heart disease, and dementia.
• Wound Care:
  Skilled in wound assessment and treatment, promoting healing and preventing complications.
• Fall Prevention:
  Identify residents at risk for falls and implement strategies to reduce fall incidents.
• Knowledge of CMS Regulations and NCQA HEDIS Guidelines.
• Document accurate medical records to ensure compliance with OSHA and other regulations.
• Collaborate with the interdisciplinary team.
• Analyze and organize the care of critically complex patients.

• Minimum 6 months of experience in long-term care or a skilled nursing facility.
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• DEA License.
• CPR BCLS/BLS certified.
• Must hold a Master's Degree in Nursing.
• Completed a degree from an accredited Nurse Practitioner program.
• Reliable transportation.
• Must be willing to travel to different nursing homes across North Texas.
• Preferred experience in cardiology (training provided).
• Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.) (training provided).
• Must have great organizational and time management skills.
• Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day.
