Advanced Practice Nurse – Palliative
Community Healthcare of Texas, established in 1996, provides Hospice and Palliative Care Services across North Central Texas. Our mission is caring for patients with serious and terminal illnesses and supporting their families.
Position SummaryUnder the direction of the Chief Medical Officer and in accordance with CHOT policies, the Advanced Practice Nurse is responsible for collaborating with physicians and other team members to deliver palliative and hospice care services.
Sign‑On Bonus$7,500
Essential Functions- Document medical history, hospice eligibility, disease progress, functional decline, medications, and diagnostics per agency standards; communicate findings to the Medical Director.
- Consult Medical Director or designee when care plans fall outside standardized protocols.
- Review, follow, and update written care plans with interdisciplinary team.
- Ensure patient receives symptom control measures via collaboration with the IDT.
- Document care provided in the clinical record, showing disease progression.
- Attend physicians’ and other meetings as directed by Medical Director or Leadership.
- Build and nurture relationships with staff and community referral sources to grow program.
- Participate in QA/PI processes as required.
- Perform all duties within local, state, and federal regulations for palliative and hospice care.
- Perform other duties or special projects as directed.
- Registered Nurse licensed to practice as an Advanced Practice Nurse in Texas; additional accreditation from a recognized body (e.g., AANP, ANCC, ACHPN) preferred.
- Minimum 3 years nursing experience, preferably at APN level.
- Comprehensive knowledge in Medicine & Pharmacology, Pain & Symptom Control, Psychology of loss, and Hospice principles.
- Valid Texas Driver’s License.
- Proof of current automobile liability insurance.
- Proficient computer skills; experienced with Microsoft Office.
