Job Summary
Primary functions are to administer nursing care for patients in their place of residence and to coordinate care with the interdisciplinary group, patients, families and referring agencies. Assumes responsibility for and participates in the coordination of care.

Qualifications
- Graduate from an accredited Baccalaureate School of Nursing.
- Current state license as a Registered Nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license.
- Two (2) years experience as a registered nurse in a clinical care setting. Hospice experience preferred.
- Good interpersonal skills. Nursing skills as defined as generally accepted standards of practice. Specialized skills preferred in palliation and end‑of‑life. Must read, write and comprehend English.
- Reliable transportation and valid auto liability insurance.
Responsibilities
- Initial and ongoing comprehensive assessments of the impact of terminal disease on the patient’s physical, functional, psychosocial, and environmental needs.
- Apply specific criteria for admission and re‑certification to hospice care to establish appropriate levels of care and the patient’s eligibility.
- Implement and document the plan of care to ensure quality and continuity of care.
- Consult with and educate the patient and family regarding disease process, self‑care techniques, end‑of‑life care, nutrition, and dietary needs, prioritizing needs with the interdisciplinary team.
- Determine scope and frequency of services based on acuity and patient/family needs.
- Initiate preventive and rehabilitative nursing procedures.
- Apply infection control and standard precautions in coordinating and performing patient care activities.
- Provide clinical direction to Hospice Aide and LPN/LVN to ensure quality and continuity of service.
- Prepare clinical and progress notes demonstrating progress toward established goals.
- Inform physician and other personnel of changes in patient needs and outcomes of interventions.
- Monitor assigned cases to ensure compliance with third‑party payor requirements.
- Participate in hospice-sponsored in‑service programs, QAPI program and promote hospice philosophy and policies.
- Perform on‑call responsibilities and provide on‑call services to patients and families as assigned.
- Provide effective communication to patients, families and team members.
