* Summary:
** The LVN, Case Manager Liaison will coordinate efficient and effective transition for patients from hospital setting to home environment while maximizing patient outcomes.

* Education:
*** The formal education from an accredited college or university in area of nursing. Education and job training as a Licensed Vocational Nurse with current State of Texas licensure (or license eligible) is required.
* Minimum of 3 years pediatric experience required; 2 years of home health and/or discharged facilitation/case management experience preferred.
* Knowledge and understanding of pediatric home care.
** Certification/Licensure:
*** Current State of Texas licensure (or license eligible) is required.
* Basic life support (BLS) certification or scheduled attendance of a CPR certification course.
