In-Home Healthcare RN provides compassionate, skilled nursing care to patients in their homes.
Key Responsibilities- Conduct comprehensive patient assessments and monitor health status.
- Administer medications and provide prescribed treatments.
- Develop individualized care plans with physicians and an interdisciplinary team.
- Educate patients and families on disease management, safety, and self-care.
- Document all care and clinical updates in an electronic medical record.
- Build trusting patient relationships and support a patient-first approach.
- Active RN licensure and strong clinical nursing skills.
- Ability to assess patients comprehensively and adjust care within scope.
- Medication administration proficiency and treatment delivery experience.
- Excellent communication, teaching, and patient/family education skills.
- Comfort working collaboratively with physicians and care teams.
- Strong documentation skills in an electronic medical record.
