Position: Compassionate Nurse Role

We are looking for a skilled RN with a dedication to quality care and a minimum of four years of experience, specifically in community nursing. You will work closely with the Interprofessional Team to create tailored discharge plans that support client needs and foster a seamless transition back home.

Key Responsibilities:

• Assess client needs for effective discharge planning

• Collaborate with teams to connect clients to community resources

• Communicate vital information to patients and families

• Refer clients to long-term care and home support services

• Maintain high standards of care and documentation

Requirements:

• Registration with BCCNM as a practicing nurse

• Minimum of four years post-graduate nursing experience

• Two years in community care nursing required

• Strong relationship-building abilities

• Valid B.C. Driver’s License needed
