Key Responsibilities:

• Deliver compassionate patient care pre and post-discharge

• Collaborate on client discharge planning with the team

• Facilitate referrals to community healthcare services

• Work with managers to ensure continuity of care

• Adhere to professional practice standards set by BCCNM

Requirements:

• Valid Registered Nurse license from BCCNM

• Passion for quality patient-centered care

• Excellent collaboration and communication skills

• Experience in discharge planning strongly preferred

• Knowledge of community healthcare resources helpful
