** Licensed Practical Nurse
** to deliver compassionate, skilled care in patients’ homes. You’ll build meaningful relationships through one-on-one visits while working closely with an RN Case Manager and a supportive clinical team.

** What You’ll Do
*** Provide skilled nursing care according to each patient’s individualized care plan and physician orders.
* Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
* Teach patients and caregivers techniques that support health, safety, and independence at home.
* Collaborate with the RN Case Manager as patient needs and care plans change.
* Document visits, nursing interventions, and medication information accurately and promptly.
* Communicate with physicians, patients, families, and the clinical team to keep care coordinated.
* Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.

** What You’ll Bring
*** An active
** Massachusetts LPN license
*** Graduation from an accredited nursing program
* At least
** one year of clinical nursing experience
*** A valid driver’s license, reliable transportation, and current auto insurance
* Ability to travel between patient homes and assigned work locations
* Ability to sit, stand, bend, lift, and move intermittently, including lifting
** 50–100 lbs.*
