Job Duties /Responsibilities
  • Help individuals identify and develop interests and hobbies. Facilitate community relationships and friendships aligned with individuals’ interests.
  • Empower individuals to advocate, make decisions, and become more independent at home and in the community.
  • Assist with personal care tasks including toileting, bathing, grooming, hygiene, mobility, dressing, and range of motion exercises.
  • Support with Activities of Daily Living (ADL) such as cooking, cleaning, shopping, and money management.
  • Use prescribed supportive and adaptive health-related equipment.
  • Assist in developing and implementing Individual Service Plans (ISP), including behavioral and medication plans.
  • Follow the Department of Public Health’s Medication Administration Program (MAP) policies, including training, medication administration, security, and documentation.
  • Utilize Positive Behavior Supports (PBS) to create a supportive environment reflecting individuals’ preferences and goals.
  • Learn and use communication tools suited to individual needs, including email, tablets, and letter boards.
  • Assist with community access and transportation for activities like day programs, medical appointments, and recreation.

Job Requirements

To perform this role successfully, you must be able to perform all essential duties satisfactorily. Requirements include:

  • Knowledge of working with individuals with developmental and/or physical disabilities.
  • Proficiency in English, both written and verbal.
  • Ability to interpret assessments and medical treatments.
  • Safe driving abilities.
  • Interpersonal skills.
  • Proficiency with Microsoft Office products.
Education/Licensure Requirements
  • High school diploma or equivalent.
  • Certified Nursing Assistant, Associate’s, or Bachelor’s degree preferred.
  • Valid driver’s license with a satisfactory record and at least 2 years of driving experience; proof required upon hire and as requested.
  • If using personal vehicle for work, valid auto insurance coverage is required.
Physical Requirements
  • Ability to lift, push, pull, and maneuver 25-75 pounds.
  • Ability to reach, bend, stand, sit, and walk.
  • Repetitive movements of wrists, hands, and fingers.
  • Close visual acuity for data analysis, reading, and computer work.
  • Ability to work in a moderate to loud environment.
