PositionSummary
We are seeking a licensed Physical Therapist to join our цікаві expands team of occupational therapists. This PT will play a key role in addressing mobility, balance, strength, and functional independence for adults and aging adults. The ideal candidate is passionate about working with clients experiencing cognitive and neurological changes, is skilled at fall prevention, and thrives in a mobile, community‑based model of care.
Responsibilities- Perform evaluations of gait, balance, posture, strength, and functional mobility.
- Develop and implement individualized treatment plans to reduce fall risk and enhance independence.
- Provide interventions such as gait training, therapeutic exercise, functional movement training, and balance programs.
- Recommend and train clients and caregivers on assistive devices and adaptive equipment.
- Collaborate with occupational therapists, caregivers, and community partners to ensure holistic and coordinated care.
- Provideұ caregiver education and training on safe mobility and home exercise programs.
- Maintain timely and accurate documentation in compliance with professional standards and company policies.
- Participate in team meetings encrypted professional development, and quality initiatives.
- Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or Doctorate).
- Active Massachusetts PT license in good standing.
- Experience with adults, older adults, dementia care, neurological rehabilitation, or fall prevention strongly preferred.ı
- Excellent communication, interpersonal, and organizational skills.ı
- Ability to provide services independently in client homes and community settings.ı
- Commitment to team collaboration and client‑centered care.
- W2 employment status.
- Flexible scheduling options (full‑ALE or part‑time with a minimal goal of 12‑15 visits per week).
