Hospice of The Bluegrass, Inc. seeks a dedicated PACE Licensed Practical Nurse (LPN) to provide direct nursing care and supportive clinical services across the PACE Center, home, hospital, nursing home, and assisted living settings.
Under RN and PACE leadership, you will administer medications, observe and report changes in condition, document care, coordinate services, and communicate with the interdisciplinary team to promote safety and high-quality care for participants and their caregivers.
