- The physician serves as the attending physician for residents at each facility.
- Collaborate with Nurse Practitioners and Physician Assistants to manage day‑to‑day resident care.
- Provide high quality, patient‑centered, comprehensive primary care services by rounding on patients residing in SNFs.
- Diagnose and treat patients with acute illnesses (e.g., bacterial and viral infections) as well as chronic diseases (e.g., hypertension, heart disease, diabetes).
- Complete provider documentation in Theoria’s proprietary EHR system, Chart Easy™ and other facility EHR systems (Point Click Care , Matrix Care, American Health Tech).
- Participate in Theoria’s Chronic Care Management, Transitional Care, RPM and other medical quality initiatives and programs.
- Supervise collaborating nurse practitioners and physician assistants.
- Utilize electronic/online medical tools such as EHR systems and state/county medical systems for administrative functions.

ShiftStructure
- Shifts are flexible depending on physician schedule.
- Typically, attending physicians work a minimum of 1‑2 days per week at each facility they oversee.
- Rounding times are flexible with no set start and end time; prefer starting between 7‑10 a.m. and finishing when caseload is seen.
- Call is covered by Theoria’s virtual care team.
- This is an in‑person position.
