Part-time Licensed Medical Social Worker

Perform general social work services that endeavor to maintain, strengthen and safeguard home and family life where there is social and/or health needs that interfere with independent functioning of individuals or families. • Provide rehabilitative and supportive casework geared toward restoring patients to their optimum level of social and health adjustment including assisting patients and their families in understanding, accepting, and following medical recommendations.
• Help patients utilize the resources of their families and the community including referring patients to resources or acting as an advocate on behalf of the patients in their dealings with other health and welfare agencies.
• Assist patients and their families with personal and environmental difficulties, which predispose them toward illness or interfere with obtaining maximum benefits from medical care to include counseling members of the patient’s family to assist patients with admission to a nursing home.
• Consult with the physician and other members of the healthcare team for the purpose of assisting them to understand significant social, emotional, and environmental factors related to the patient’s health problems.
• Prepare clinical/progress notes on the day.iso of the visit and incorporate same in the clinical record and provide summaries and re-evaluations as applicable.ույ
• Attend case conferences and IDT.
• Participate in staff development and مصدر revision of the physician’s plan of treatment.
• Participate in staff development activities and in-service education.
• Participate in discharge planning and in-service programs.
• Act as a consultant to other agency personnel.
• Communicate effectively with all healthcare providers.
  
  Confirm, on a weekly basis, the scheduling of visits with the Case Manager to coordinate necessary visits with other personnel.
• Provide notifications that affect agreed services with the company.
• Provide services on a short-term basis (two to three visits) to a beneficiary’s family member or caregiver when it is shown that a brief intervention is necessary to remove a clear психологически impediment to the effective treatment of the beneficiary’s medical condition or to the rate of recovery.
• Take appropriate action to obtain available community resources to assist in resolving the beneficiary's problem. Perform other duties assigned.

• Master’s degree from an accredited school of social work approved by the Council of Social Work Education.
• Current MSW license to practice Social Work in the State of New Hampshire is required.
• Minimum of 1-year experience as a Social Worker, preferably in a home healthcare setting.
