This position offers a competitive salary with a $5,000 sign‑on bonus payable after one year of service.

- Interview clients/families to obtain a comprehensive history and developmental status.
- Conduct physical examinations to gather objective data.
- Review clinical data, including referrals and lab results.
- Complete physical and psychosocial assessments for new or ongoing visits.
- Document data and assessments on appropriate forms following agency guidelines.
- Assess need for palliative care and refer appropriately.
- Reassess client status at scheduled intervals and report changes.
- Identify potential crisis situations and develop individualized care plans.
- Collaborate with interdisciplinary teams to create integrated care plans.
- Initiate discharge planning as part of the care plan.
- Coordinate medical diagnostic and therapeutic orders.
- Promote client dignity and provide a calm, supportive environment.
- Educate staff, patients, and families regarding care and resources.
- Serve as a primary case manager when requested.
- Perform other duties as assigned.
Education and Experience
- Bachelor of Science in Nursing (BSN) or equivalent from an accredited program.
- Current RN license in the State of New Hampshire.
- Minimum 1 year of progressive professional experience, preferably in home‑care.
- Deliver age‑appropriate client care and adapt care plans to meet diverse needs.
- Communicate clearly through spoken and written instructions.
- Plan and execute diversified nursing duties with sound judgment.
- Work independently following standard practices and refer questionable cases.
- Demonstrate courtesy, compassion, and respect toward patients, families, and colleagues.
- Maintain positive working relationships with healthcare professionals.
- Document patient care accurately per nursing standards.
- Proficiency with Microsoft Office and other relevant software.
- Follow agency safety guidelines and wear protective clothing as required.
- Travel throughout central New Hampshire; valid driver’s license and automobile insurance required.
- Pass a motor vehicle background screening.
- Stand for extended periods; bend, lift (up to 50 lbs occasionally, 30 lbs regularly), reach, kneel, push, pull, crouch, crawl, grip, and perform tasks requiring moderate force.
- Maintain close visual acuity; see at or within arm’s reach.
- Work in a low‑noise indoor environment.
