Concord Hospital – Franklin 2 South is a 25-bed critical access facility that cares for the people of Franklin and the surrounding areas. Our 14-bed inpatient medical unit provides short‑term subacute skilled care and rehabilitation services, including physical, occupational, speech, respiratory, and recreational therapy. The unit sees patients recovering from orthopedic surgery and medical events. It is staffed daily by two nurses, two LNAs, a medical provider, and rehabilitation services.

We are a close-knit community where everyone knows each other’s names and works together to achieve the best outcomes for our patients. The team has 6 months‑to‑22 years of experience on the unit and we are currently growing our program to offer continued improvement and advancement in your career. We hope you consider joining us.

Education

Graduated from an accredited School of Nursing, BSN preferred.

Certification, Registration & Licensure

Licensure required:
Current New Hampshire State Registered Nurse license.

Certification required:
Current American Heart Association Basic Life Support for Healthcare Providers certification, or equivalent credentialed by the American Heart Association.

30 hours a week

Related Experience/Training:
Evidence of experience in working effectively and positively with patients, care providers, and the multi‑disciplinary team. Evidence of the ability to perform as a member of a team.

Responsibilities
  • Bases nursing actions by applying principles of purposeful, organized thinking to make clinical judgment/decisions.
  • Gathers data through history taking and physical assessment and interprets data in order to plan and implement interventions to meet patient care needs.
  • Develops, evaluates and revises patient’s plan of care and nursing actions in response to patient’s needs.
  • Documents care provided and patient’s progress toward attainment of desired outcomes.
  • Directs and supervises others in the performance of delegated nursing activities.
  • Teaches the patient and/or significant other(s) about the plan of care. Prepares patients to manage their care after discharge, transfer or preparation for procedure/test.
  • Manages time and resources to deliver patient care.
  • Administers medications to patients.
  • Provides direct patient care.
  • Leads the health care team.
  • Performs job specific requirements and other duties as assigned.

The employee is occasionally exposed to airborne contaminants, chemotherapeutic agents, electrical hazards—shock, moving mechanical parts, non‑weather related heat or cold, slippery surfaces, and toxic or caustic chemicals.
