Aboutthe Facility
Ahava Therapy Services is hiring a PRN Physical Therapist Assistant for Franklin Wellness & Rehab, an 83-bed skilled nursing facility providing both short-term and long-term care. Therapists at Franklin work as part of a collaborative in-house rehab team, with strong leadership support, to help residents achieve independence and optimal function.
Responsibilities- Conduct thorough functional assessments and develop individualized treatment plans.
- Deliver and document evidence-based therapy sessions.
- Supervise and instruct therapy assistants as needed.
- Collaborate with nursing staff, residents, and families to maximize functional abilities.
- Educate residents and caregivers on home exercise and therapy continuation.
- Maintain compliance with documentation, safety, and quality standards.
- Participate in interdisciplinary meetings and continuous improvement initiatives.
- Maintain professional knowledge through ongoing education.
- Bachelor’s or Master’s degree in Physical Therapy
- Current PT license in the state of practice
- BLS/CPR certification (current or obtained during orientation)
- Strong teamwork and organizational skills
