Bluestone is seeking a Nurse Practitioner or Physician Assistant to provide on-site, team-oriented primary care in Assisted Living and Memory Care communities around Oak Creek, Franklin and Milwaukee, WI. This full-time role features a 200-patient panel and a schedule designed for a balanced mix of clinical and administrative days.
You will work with LPNs, RNs, and social workers, access onsite diagnostics, and develop individualized care plans.
