The Resident Care Coordinator is responsible for coordinating and overseeing resident care to ensure each individual maintains the highest level of independence, dignity, and quality of life possible. This role partners closely with residents, families, healthcare providers, and internal team members to support both the physical and emotional well‑being of residents while ensuring care services are delivered in compliance with all state, federal, and company standards.
ResidentCare Coordinator
Key Responsibilities Resident Care & Coordination
- Coordinate pre‑move‑in, move‑in, and ongoing resident assessments
- Assist in developing, implementing, and monitoring individualized care plans
- Serve as a key point of contact for residents and families regarding care needs
- Monitor changes in resident condition and communicate appropriately with leadership and care teams
- Liaise with physicians, hospitals, pharmacies, and community healthcare providers
- Assist with hiring, training, scheduling, and supervising care team members
- Support coaching, performance feedback, and ongoing staff development
- Ensure appropriate staffing levels to meet resident needs and regulatory requirements
- Promote teamwork, accountability, and alignment with Solera’s Core Values
- Coordinate prescription orders and pharmacy services
- Support proper medication storage, administration, documentation, and compliance
- Ensure team members administering medications are trained and compliant
- Ensure compliance with all federal, state, and company regulations
- Participate in onboarding and ongoing training for care team members
- Support survey readiness, audits, and quality assurance initiatives
- Maintain a safe, clean, and secure environment for residents and team members
- Assist with emergency preparedness, safety procedures, and incident reporting
Maintain inventory of care‑related supplies and equipment
- Support tours, resident transitions, and census growth initiatives
- Foster positive relationships with residents, families, and community partners
- Contribute to a welcoming, professional, and resident‑centered community culture
- Preferred:
Two (2) years of college coursework in healthcare, human services, or a related field - Preferred:
Four (4) years of experience in senior living, residential care, or dementia‑specific programming
- Strong leadership, organization, and decision‑making skills
- Excellent verbal and written communication skills
- Ability to manage staffing, schedules, and departmental operations
- Comfortable working in a fast‑paced, resident‑focused environment
- Proficient with basic office systems and electronic documentation
- Compassionate leader with a passion for dementia care
- Demonstrates integrity, accountability, and teamwork
