Full-Time | Premier Senior Living Community | Frederick, Maryland Prava Therapy THAT HELPS PEOPLE KEEP LIVING LIFE ON THEIR TERMS.
Prava Therapy is seeking a full-time Physical Therapist Assistant (PTA) to join our team within a premier, luxury resort-style senior living community offering Assisted Living and Memory Care in Frederick, Maryland.
This community emphasizes a holistic wellness philosophy—focusing on an active lifestyle, personalized care, life enrichment, resort-style amenities, and continuous engagement. Residents value independence, connection, health, and maintaining an active, fulfilling life.
You'll get to know your residents, their routines, and the activities that matter most to them.
YOUR ROLEAs a Physical Therapist Assistant, you'll deliver skilled clinical treatments under the direction of a Physical Therapist, bringing PT plans of care to life while embedding yourself as a trusted partner within this vibrant community.
You'll:
Implement individualized, evidence-based PT treatment interventions according to established plans of care across adult/geriatric physical therapy domains.
Carry out treatments addressing strength, balance, gait training, transfer mechanics, functional mobility, endurance, joint integrity, and active daily routines.
Target fall prevention, balance recovery, mobility device training, apartment safety, and environmental navigation across both Assisted Living and Memory Care settings.
Introduce, adapt, and train residents, caregivers, and staff on mobility equipment, assistive devices, compensatory movement strategies, and physical safety tools.
Leverage community features—such as fitness centers, life enrichment spaces, outdoor pathways, and therapy spaces to practice real-world functional mobility and engagement.
Identify subtle changes in resident function, safety, or physical status to support proactive care, communicating regularly with the supervising PT.
Educate residents, families, caregivers, and community staff on functional independence, fall risk reduction, safe transfers, and mobility support.
Collaborate with supervising Physical Therapists, interdisciplinary team partners, community leadership, wellness directors, life enrichment teams, and caregivers.
Monitor resident progress, document treatment sessions accurately and in a timely manner, and provide feedback to the PT for plan-of-care updates.
Participate in care coordination, interdisciplinary team discussions, and community wellness initiatives as appropriate.
Contribute to a culture of clinical excellence, compassion, professionalism, and continuous learning.
You believe great therapy isn't simply about guiding someone through exercise repetitions or completing a routine checklist. It's about helping someone participate fully in the lifestyle and community they've chosen.
That might mean walking confidently to dining venues, participating in life enrichment events, navigating community amenities independently, participating in group activities, or managing daily routines effortlessly.
You believe:
Every intervention should have a functional, real-world purpose.
Residents should be appropriately challenged—not underestimated.
Independence, autonomy, and holistic well-being are clinical priorities.
Proactive clinical identification, mobility adaptation, and safety support matter.
Assistive device integration, routine structuring, and caregiver education are essential components of skilled care.
Functional outcomes matter more than simply completing treatment minutes.
Strong clinical relationships lead to better care.
