Stern Therapy Consultants is hiring a Physical Therapist for PRN coverage at Autumn Lake at Ballenger Creek. Support residents through short-term recovery and ongoing rehabilitation, with treatment that connects improved strength and mobility to everyday tasks in a skilled nursing setting.

• Complete physical therapy evaluations to identify mobility limitations, establish goals, and develop individualized plans of care.
• Deliver treatment addressing walking, balance, strength, endurance, and safe transfers.
• Select appropriate mobility devices and teach residents how to use them during functional activities.
• Review residents’ progress and modify treatment plans based on clinical findings and response to therapy.
• Collaborate with nursing, families, and the rehab team on mobility recommendations and discharge needs.
• Maintain accurate clinical records and supervise PTA services in accordance with the plan of care.

• $55–$60 per hour.
• PRN coverage opportunities based on facility needs and your availability.
• Unlimited referral bonus, with up to $1,000 per successful referral.
• Clinical collaboration and support from the rehabilitation team and therapy leadership.

• Active Maryland Physical Therapist license.
• Strong assessment, treatment planning, and documentation skills.
• Ability to communicate care recommendations clearly and manage a varied rehabilitation caseload.
• Skilled nursing experience is a plus; new graduates are welcome to apply.
