Adoration Home Health and Hospice seeks a Licensed Practical Nurse to join our team in Westminister, MD. As a Home Health LPN you will provide patient-centered care in the comfort of patients’ homes.

• Providing 1:1 care following the client’s plan of care while maintaining dignity and independence.
• Administering medications, feedings, oxygen, ostomy care as needed.
• Accurately documenting all nursing actions, communicating with family, case manager, physicians, and documenting progress and outcomes for established goals.
• Coordinating or ordering supplies, medications, and feedings to ensure adequate supplies.
• Ensuring all patient needs are met in accordance with physician instructions.

• LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record, proof of car insurance.
• CPR professional certification.
• 1+ years nursing experience preferred.
• Home Health care experience preferred.
