• Assess patient needs and develop individualized recovery plans.
• Deliver physical therapy services in facility settings.
• Educate patients and caregivers on skills to promote independence.
• Collaborate with peers and refer patients for additional care as needed.
• Operate as a teammate contributing to a culture of patient-centered care.

• Valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or related field, or Bachelor's degree with five years of progressively responsible PT experience.
• Ability to work in a facility environment.
• Strong clinical assessment and treatment planning skills.
