Responsibilities

• Guide and counsel individuals and families in crisis, including unemployed individuals, foster care cases, and elderly clients with limited support
• Assist clients in obtaining government funds, education, or treatment
• Secure and evaluate information contributing to the client’s situation, including medical, psychological, and social factors
• Explain problems to clients to determine the nature and degree of needs; propose plans for meeting needs
• Help patients and families through individual or group conferences regarding plans to meet needs
• Assist clients in modifying attitudes and behavior by increasing understanding of self and situations
• Develop a service plan to restore the patient to optimum social and health adjustment within their capacity

Qualifications

• Master’s Degree in Social Work
• Five plus years of clinical social work experience
• Valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license
• Ability to make psycho-social assessments and develop and implement viable care plans
