1) increase access to community-based mental health and substance use disorder services (particularly to under‑served communities),
2) advance integration of behavioral health with physical health care, and
3) improve utilization of evidence‑based practices on a more consistent basis.

The School-Linked Behavioral Health Practitioner and Targeted Case Manager plays a vital role in supporting students' emotional and behavioral well‑being. This position is responsible for providing skills training, targeted case management, crisis intervention, and treatment plan development and review. The practitioner also administers and reports standardized assessments to monitor progress and guide care. Working closely with school staff, families, and community partners, the practitioner ensures coordinated, student‑centered support aligned with individualized treatment goals.
This role may be based in contracted schools, RMHC office sites, or the community/home of the client and may require travel between sites.
- Provide individual, group, and family skills training at assigned school site(s), clients’ homes, or within the community.
- Instruct, assist, empower people served to develop and enhance stability, competencies, personal/emotional adjustments and independent living and community skills.
- Provide direct client case management support as assigned.
- Complete prior authorizations as needed for services; ensure insurance is active, and assigned clients are eligible for services monthly.
- Assist with scheduling and compliance with regards to appointments.
- Coordinate School-Linked Behavioral Health Services at assigned school site(s).
- Build and maintain positive working relationships with partnering agencies.
- Provide school/community personnel with essential program information.
- Respond to and provide crisis assistance and intervention as needed.
- Provide consultation and educational services to school/community authorized people.
- Complete all clinical documentation and other medically necessary information in the RMHC electronic health record for the people we serve both as required by agency policy and DHS standards in a timely manner.
- Develop and update individual treatment plans as required.
- Complete functional assessments (FA), Individual and Family Community Support Plans (IFCSP), and crisis prevention plans (CPP) as required.
- Complete required standardized measurement tools for children and adolescents.
- Provide medically necessary behavioral health treatment to children, adolescents and their families, as directed by RMHC Code of Conduct and Minnesota regulations.
- Attend Multidisciplinary, Special Education, school, and other meetings as directed.
- Coordinate care and referrals with other service agencies and families as needed for the people we serve.
