Community-Focused Registered Nurse

Contribute to public health initiatives with Horizon as a Community-Focused Registered Nurse. Engage directly with individuals and groups to promote health and prevent diseases.

Horizon is searching for a Registered Nurse Class A with a Bachelor’s degree and active registration from CNNB. Applicants should have at least one year of public health experience or three years in community health nursing. Work collaboratively with community partners to implement effective health programs.

Key Responsibilities:

• Manage nursing caseload and prioritize patient needs

• Develop and assess health service plans collaboratively

• Participate as a key member of an interdisciplinary team

• Maintain documentation in line with standards

• Cultivate partnerships with healthcare providers and community organizations

Requirements:

• Bachelor’s degree in nursing required

• Active CNNB registration and public health experience

• Knowledge of community health strategies

• Excellent communication and leadership skills

• Proficient with Microsoft Office applications
