Case Manager for Urban Health Services

Become a Case Manager with The Neighbourhood Group's Urban Health and Homelessness Services, focusing on the Mobile Shelter Support Program. This full-time position supports vulnerable adults in achieving their personal goals and improving their lives.

This role involves intensive case management for clients dealing with complex issues across different services and supports. From coordinating health services to addressing housing concerns, you will provide individualized support and ensure clients can access needed community resources. By utilizing a trauma-informed approach, you will play a pivotal role in guiding participants towards stability and success.

Key Responsibilities:

• Conduct intake interviews and detailed assessments

• Provide ongoing support through home and community visits

• Develop partnerships with external service providers

• Monitor client housing and property issues as needed

• Engage in crisis intervention and team management meetings

Requirements:

• BA/B.S.W alongside 2 years of case management experience

• Direct experience with homelessness and mental health issues

• Familiarity with trauma-informed care practices

• Strong communication skills in English

• Knowledge of MS Office and data entry for case management
