Essential Functions
  • Manages primary caseload of Hospice patients per physician's instructions and in compliance with regulations
  • Must do on-call rotation
  • Travel to Applewood Village and Pines Village
  • Implements and updates care plans to address patient's nursing needs
  • Educates and supports patients, family, and staff on nursing care and patient needs
  • Collaborates with the Interdisciplinary Team (IDT) to meet all patient and family care requirements
  • Supervises Licensed Practical Nurses and Hospice Aides when needed
  • Some on-call responsibilities with additional financial compensation
Qualifications
  • Active and valid NJ RN license
  • Preferred certification:
    Certified Hospice and Palliative Nurse (CHPN)
  • Preferred minimum of 2 years experience in hospice or palliative care but willing to train the right person
  • Ability to collaborate within an interdisciplinary setting
  • Knowledge and understanding of hospice philosophy and principles of death/dying
  • Valid driver's license and reliable, insured transportation in accordance with New Jersey state requirements

  • Exceptional communication and interpersonal skills
  • Robust critical thinking and problem-solving abilities
  • Capable of working independently and collaboratively in a multidisciplinary team
  • Proficient in electronic medical record systems and relevant software applications
